We assist minority and low-income residents to complete Health Risk Assessments (HRA), book annual wellness visits, take preventative health screenings, complete redeterminations, and address the Social Determinants of Health (SDoH). Our teams drive health equity in Baltimore, DC, Los Angeles and other underserved markets.
