Sarah Chouinard, M.D. has spent her career as a rural primary care physician and currently serves as Chief Medical Officer of Main Street Health, the largest rural value-based care company in the United States. Prior to Main Street Health, Dr. Chouinard was the Chief Medical Officer for Community Care of West Virginia, the second-largest FQHC in West Virginia, for 15 years.
