When you think about health, which is more powerful? a person?s ZIP code or genetic code? To meet the social needs of patients, providers need to be aware of the risks each population faces, adjust clinical care to address those needs, and connect patients with community resources. Home-based care plays a unique role in identifying and addressing these SDoH factors. How can we better integrate SDoH care into home-based care models with virtual as an enabler?
