About this Session:

This panel discussion will feature panelists from across the care continuum. Each panelist will provide their view on the key pillars in developing an integrated care system (ICS) in the digital age to keep people healthy through seamless, convenient care and wellness services. Ideally, these systems will provide equitable, well-connected and coordinated care across all aspects of a person?s health and social needs resulting in healthier populations and more sustainable systems. The discussion will consider observations over three key pillars: 1) Infostructure and core features of a user-centric, clinically governed, interoperable, vendor-neutral, safe, open and easy-to-use digital environment 2) Patient and clinician experience and operating model design attributes to drive adoption, financial and operational sustainability, and engagement 3) Organizational change, workforce and incentive strategies to enable transformation and new models of care to drive better health outcomes Learn more about developing this path forward to innovate the care models into ICSs which offer anytime, anywhere holistic care and well-being for all.
