This panel discussion will feature panelists from across the care continuum. Each panelist will provide their view on the key pillars in developing an integrated care system (ICS) in the digital age to keep people healthy through seamless, convenient care and wellness services. Ideally, these systems will provide equitable, well-connected and coordinated care across all aspects of a person?s health and social needs resulting in healthier populations and more sustainable systems.
