In 2022, the California Dept. of Health Care Services, California Primary Care Association, and California Community Health Centers partnered with Kaiser Permanente to launch the Population Health Management Initiative (PHMI). PHMI will address disparities and care quality related to screening and preventive care for chronic conditions, maternal and child health, and psychosocial wellbeing among California Medi-Cal enrollees. Health care resources and connections will be provided to 32 FQHCs, representing 8 counties and 1.1 million people. This discussion will focus on the collaborative efforts in developing and implementing the PHM I model, as well as the approach taken to sharing knowledge and data around social determinants of health.
