Aledade Expands Value-Based Care Network with 500 New Primary Care Practices

Aledade, the country's largest network of independent primary care providers, has announced a significant expansion with the addition of more than 500 new primary care practices, clinics, and community health centers to its network. This growth extends the company's reach to over 2,400 healthcare organizations serving nearly 3 million patients through value-based care programs across the United States.

The expansion demonstrates substantial progress in advancing value-based care models, particularly within the Medicare Shared Savings Program (MSSP). Notably, Aledade is driving new participation in value-based care initiatives, with 83% of its new MSSP participants entering the program for the first time. The company now represents nearly one-fifth of all new practices joining the MSSP.

"Since day one, Aledade has led the charge in enabling independent primary care doctors nationwide to adopt and thrive in value-based care, enhancing patient outcomes while ensuring practice sustainability," said Farzad Mostashari, M.D., co-founder and CEO of Aledade.

The network's growth has been strategically focused on increasing healthcare access across diverse communities, including partnerships with over 300 community health centers, many serving underserved areas. This approach aligns with Aledade's mission to transform healthcare delivery while supporting independent practices.

Operating across 46 states and the District of Columbia, Aledade's expanded network represents a significant milestone in the shift toward value-based care models. The company's platform provides essential support and resources to help independent practices transition to and succeed in value-based care arrangements, ultimately working to improve patient outcomes while ensuring the sustainability of independent primary care practices.
