Cityblock Health, a value-based care provider focused on serving Medicaid populations, has formed a strategic collaboration with Mercy Health, part of the Bon Secours Mercy Health system, to improve care delivery in the greater Cincinnati and Toledo areas of Ohio. Under this partnership, Cityblock will extend Mercy Health’s primary care services through a wrap-around care coordination model that operates 24/7 and integrates primary and urgent care, behavioral health, and social care services across both in-person and virtual settings.
Dr. John Luellen, State President for Mercy Health in Ohio, said the collaboration enhances the organization’s ability to meet the evolving needs of its communities by expanding access to compassionate, high-quality care for Medicaid patients and addressing their full spectrum of needs.
The partnership aligns two mission-driven organizations dedicated to improving outcomes for underserved populations. By combining Mercy Health’s provider network with Cityblock’s community-based care model, the initiative seeks to better engage hard-to-reach patients, improve access to essential care and social services like food and housing, and reduce unnecessary emergency room visits and hospitalizations.
Cityblock President Mike Roaldi noted that the collaboration builds on Mercy Health’s existing work by extending care beyond the doctor’s office and into patients’ homes and communities, ultimately aiming to deliver more holistic and cost-effective care.
