Conduit Health Raises $17M Series A Led by Drive Capital to Expand AI-Powered DME Access Platform

Conduit Health has raised $17 million in a Series A funding round led by Drive Capital, with participation from existing investors including Carrot Fertility, Sidecar Health, and Sondermind. The financing brings the company’s total funding to $22 million and will support expansion of its technology, operations, and partner network.

The company focuses on simplifying access to durable medical equipment (DME), a process that remains complex despite many patients qualifying for coverage through Medicare, Medicaid, or managed care plans. Administrative barriers such as prior authorizations, documentation requirements, and coordination across providers often delay or prevent access to essential home medical supplies.

Conduit Health addresses these challenges through an integrated platform that consolidates multiple steps into a single workflow. Its model coordinates clinical evaluation, insurance verification, authorization, and equipment fulfillment, reducing fragmentation and shortening delivery timelines.

At the core of the platform is CareOS, an AI-driven authorization and reimbursement engine trained on more than 50,000 patient interactions. The system analyzes payer requirements across Medicare, Medicaid, and commercial plans, enabling prediction of approval likelihood before claims submission and automating key administrative processes.

Natan Wise, co-founder and CEO of Conduit Health, said, "More broadly, the Series A will allow us to expand our team, technology and operations so we can reach significantly more patients across the country."

He further explained the company’s approach, stating, "Success for us means removing the cognitive burden from patients and caregivers."

Since launching publicly in late 2024, Conduit has scaled its operations, delivering supplies to more than 50,000 patients and building a payer network spanning over 100 health plans and nearly 90 million covered lives. The company has also expanded its product catalog and increased utilization among referral partners, including home care agencies, skilled nursing facilities, and case managers.

She added, "Conduit’s vertically-integrated model brings those steps into one system, which allows them to reduce delays and create a much more predictable, hands-off experience for patients."

Looking ahead, Conduit aims to extend its platform beyond DME to support broader home-based services, including transportation, nutrition, and home modifications, as healthcare delivery increasingly shifts into the home.
