Atlanta-based Emory Healthcare is scaling its population health management efforts, aiming to serve over 350,000 patients across Georgia. Partnering with health tech company Guidehealth, Emory has launched a population health collaborative to enhance primary care, integrate its physician network, and improve care outcomes.
The collaborative will support more than 500 primary care providers across Emory’s network of 490+ locations, focusing on preventive care, chronic disease management, and patient engagement. Guidehealth’s AI-driven tools and virtual clinical teams will streamline care coordination, reduce administrative burdens, and enhance predictive modeling to identify at-risk patients early.
Patrick Hammond, CEO of Emory Healthcare Network and the new collaborative, highlighted the strategic investment as critical to scaling Emory’s population health efforts. Guidehealth's platform enables integration across diverse electronic medical records and payer classes, enhancing value-based care performance while reducing operational risks.
Guidehealth, founded in 2022, brings advanced analytics and AI tools to support health systems in risk-based contracts. CEO Sanjay Doddamani emphasized the company’s role in improving patient engagement and providing embedded virtual clinical staff, called Healthguides, to assist with care delivery.
The partnership is designed to advance Emory’s shift to value-based care, enabling providers to improve quality metrics and manage costs effectively. The initiative also addresses challenges like administrative burdens on physicians and the nationwide shortage of primary care providers.
With Guidehealth’s tools, Emory aims to align clinical workflows with value-based goals, prevent complications, and improve overall health outcomes. Both organizations are actively implementing the model across Emory’s practices and network.
