Harmonic Health, a tech-enabled dementia care platform, has launched and partnered with Stoney Batter Family Medicine, a medical practice across Delaware, Maryland, and Pennsylvania. Harmonic Health, based in St. Louis, is a company affiliated with Redesign Health, dedicated to aiding digital health startups in their development, launch, and funding.
Harmonic Health offers comprehensive care management for patients and their caregivers, featuring a physician-engagement platform and around-the-clock behavioural, social, and medical support. Their services encompass telehealth, predictive care, and educational content.
Through its collaboration with Stoney Batter Family Medicine, the company will initiate a program that delivers dementia screening, caregiver assistance and coaching, educational resources, and referrals to community services for physicians and their patients.
CEO and co-founder of Harmonic, Jim Gera, emphasised the inadequacy of current neurodegenerative disease patient care, underscoring the lack of support for patients and caregivers, as well as the resource constraints for physicians. Harmonic Health's mission is to bridge these gaps in care, offering support with empathy and candour, while focusing on disease-specific care.
