UnityPoint Health and HealthSnap, a prominent virtual care management platform, have announced the extension of their partnership following successful clinical outcomes. The collaboration, initiated in January 2023, involves the implementation of HealthSnap's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs across 94 Internal Medicine and Family Medicine clinics, reaching over 25,000 UnityPoint Health patients in Iowa and Illinois.
HealthSnap's virtual care management aims to keep patients connected to their care teams between office visits, focusing on proactively managing chronic conditions like uncontrolled hypertension, heart failure, type II diabetes, and obesity.
The partnership has demonstrated significant improvements in patient outcomes, including a 50% reduction in patients with uncontrolled Stage 2 Hypertension and notable reductions in blood pressure for hypertensive patients. Fasting blood glucose levels also witnessed a substantial decrease among patients struggling with poor glucose control.
UnityPoint Health, serving nearly 8 million patient visits annually, aims to provide more personalised and accessible care. Patricia Newland, MD, President of UnityPoint Clinic, emphasised the positive impact of RPM and CCM on patient outcomes and engagement across Family Medicine and Internal Medicine practice areas.
The successful outcomes have led to the expansion of the program to five regions across Iowa and Illinois, with 409 healthcare providers offering HealthSnap's RPM and CCM programs to patients with chronic conditions in 94 UnityPoint Health clinics by the end of 2023. HealthSnap's CEO, Samson Magid, expresses optimism about continuing to achieve impressive patient outcomes as the program expands further across the Midwest.
