The pilot program, which utilized a virtual care model, yielded impressive results, including an 80% success rate in optimizing guideline-directed medical therapy (GDMT) without clinic visits and a readmission rate significantly lower than the national average. Additionally, the program improved patient engagement, quality of life, and clinic efficiency. Building on these successes, the partnership has now extended to include a hypertension management program launched in April 2024, which seeks to enhance blood pressure control among patients.
