The acquisition, facilitated by a $47.5 million investment from S2G Investments in partnership with TT Capital Partners, creates a comprehensive solution for addressing social determinants of health among vulnerable populations.
Arizona-based Pyx Health operates a mentor-driven care navigation platform that targets non-medical issues affecting vulnerable populations, including loneliness and support needs. The company provides research navigation, care coordination, and virtual care support services to Medicaid, Medicare, managed care organizations, and community health plans. FarmboxRx partners with healthcare providers to deliver fresh produce, pantry essentials, and healthy cooking kits to Medicare and Medicaid members through their insurance benefits.
The strategic combination aims to tackle food insecurity while enabling health plans to incentivize specific member behaviors, including completing health assessments and scheduling preventative care visits. This integrated approach addresses multiple social determinants of health through a single platform, potentially improving outcomes for underserved populations.
"Somebody's zip code or income should never determine the status of their health, but too often it does. In March, the company launched Drivers Health, a platform addressing healthcare needs across the social continuum. The company's Maternal Health Program, launched last year, provides nutrition access and education to at-risk pregnant women during pregnancy and postpartum periods, focusing on immunizations, prenatal care, mental health, and government support programs.
The company has also demonstrated innovation in responding to policy changes affecting vulnerable populations. In 2023, FarmboxRx launched Feed by FarmboxRx, an application-based program providing produce and pantry staples to individuals affected by SNAP benefit reductions. The company has established partnerships with major health plans, including a 2021 collaboration with Minnesota's HealthPartners to deliver farm boxes to Medicare members.
This acquisition reflects growing recognition among healthcare organizations that addressing social determinants of health, particularly food security, is essential for improving health outcomes and reducing costs in vulnerable populations.
