Switchboard Health Acquires Conduce Health to Enhance AI-Driven Specialty Care Matching

Switchboard Health, a digital health platform connecting patients with specialty care services, has acquired Conduce Health, an AI-driven solution for personalized multispecialty care. The acquisition will integrate Conduce's predictive analytics and clinical decision support technology into Switchboard's existing EMR-integrated software and virtual care network, enhancing the platform's ability to match patients with optimal specialists.

Founded in 2023 by health tech entrepreneurs and physicians and backed by AlleyCorp, Conduce developed an AI platform designed to make specialty care proactive, predictive, and personalized. The company's software enables clinical teams to refer patients to appropriate specialists based on clinical and personal variables, moving beyond traditional referral processes that often lack data-driven precision.

Through the acquisition, Switchboard Health will incorporate Conduce's AI capabilities into its referral management, care navigation, and virtual care offerings. These capabilities include predictive models for disease progression, identification of urgent medical needs, and precision patient-specialist matching algorithms. The integration aims to enable Switchboard's provider and health plan clients to more effectively connect patients with high-value specialists who are best suited to their specific clinical profiles and circumstances.

Key members of the Conduce team will join Switchboard as part of the transaction, bringing technical expertise and domain knowledge to support the integration and development of the combined platform. The acquisition positions Switchboard to deliver enhanced value-based specialty care models by applying AI-driven insights to the referral and care coordination process.
