Memorial Healthcare Launches Care Coordination Center for Patient Safety and Efficiency

Memorial Healthcare System is proud to announce the launch of its new Care Coordination Center (CCC), a 24/7 clinical command center in Pembroke Pines. Opening in October 2024, the CCC will integrate virtual operations to enhance patient safety, optimize care delivery, and improve operational efficiency across all Memorial facilities. It will focus on coordinating interfacility transfers, managing bed capacity, and staffing needs in real time while implementing virtual care models.


"At Memorial Healthcare System, we are committed to improving patient care through innovative solutions," said Elizabeth Justen, chair of the South Broward Hospital District Board of Commissioners. "The Care Coordination Center allows us to leverage real-time data and collaborative strategies to enhance patient safety and streamline care delivery."


The CCC will facilitate seamless interfacility transfers, ensuring efficient patient placement. Dedicated coordinators will oversee bed assignments to maximize capacity and improve transitions. A specialized team will monitor high-risk patients virtually, ensuring prompt interventions to reduce fall incidents. Additionally, the Virtual Nursing Initiative, launching on October 30 at Memorial Hospital West, will allow nurses to manage admissions and patient education virtually, freeing bedside nurses to focus on direct care.


The center represents a significant investment of $1.67 million, encompassing 3,000 square feet and featuring 31 workstations. It will utilize Epic EHR technology for patient flow and Artsight's AI-driven Virtual Nursing to enhance safety and satisfaction. Goals include reducing Emergency Department boarding times, decreasing fall incidents, and improving admission and discharge efficiencies.
