Post-discharge care of stroke survivors can be really challenging due to the poor management of the risk factors and medication adherence, which might increase the risk of relapse. How can we leverage digital technologies to improve the post-stroke ongoing management - reinforce understanding of new medications, encourage risk factor modification, develop strategies for change, and provide other education as needed for the patient and family? What are the main challenges in adopting these platforms from a provider and payer viewpoint? Can digital tools lower the costs of post-stroke management and avoid hospital readmissions?
