About this Session:
For generations, we?ve witnessed a widening gap in health outcomes along racial and socioeconomic lines. Communities are looking to health systems for solutions, but nearly 80 percent of our health is influenced by factors outside clinic walls. Called the social determinants of health, these include housing stability, food access, community support, and other socioeconomic factors. Health systems, along with other service providers and stakeholders, must address these social determinants where people live and work in order to close the health and wellness gap.
