PATIENTS AT HLTH IMPACT PROGRAM: Doing Right by Patients in Digital Health

Digital health tools help patients become empowered, improve chronic condition outcomes and access critical at-home care–to name a few benefits. But integrating these tools into patient care is easier said than done.
