How did an idea born out of a desire to fix a broken system become a radical care ecosystem for thousands of people, despite constantly facing intense political pressure and living at the margins in every way? Concepts such as the patient-centered medical home, health equity, population-based health, patient and provider satisfaction, and social determinants of health have been around for decades, but extremely hard to execute in practice in a meaningful way. Through using technology, the heart and soul of a beautiful, but long misunderstood community, and a vision of healthcare for all can fundamentally change how the world thinks about healthcare and the Trans and gender-diverse community. This fully virtual platform has drastically reduced wait times, integrated core gender-affirming care services with primary care, peer support groups and other social support services, and produced groundbreaking clinical research that has led to happier, healthier, more satisfied patients and care teams, and lower long-term health costs.
