Care-at-home programs are gaining momentum, driven by the need to future-proof health systems, address capacity constraints and provide a more convenient, comfortable option for patients. Health systems that adopt this model early are positioning themselves for long-term success, despite the upfront costs and operational hurdles. The business case for this is clear–shifting post-acute care to the home frees up hospital beds, strengthens patient relationships, improves adherence, and reduces readmissions. Yet, demonstrating value to payers remains a hurdle, as many still operate under a fee-for-service model and struggle to see the immediate value. For health systems, the value proposition lies in strategic resource allocation. By shifting post-acute care to the home, they alleviate pressure on inpatient facilities, freeing critical resources for higher-acuity patients. For patients, it delivers better outcomes as many prefer receiving personalized care in the comfort of their home. The key to securing payer buy-in is to demonstrate cost-effectiveness. Rather than competing with traditional hospitals, home care complements and redefines them, offering a sustainable, patient-centered approach to care. We’re all homebodies at heart, after all!
