About this Session:
Most health systems are still organized around episodic, acute care, leaving patients with advanced serious illness underserved during the most vulnerable years of life. This session will highlight how palliative and supportive care can deliver better, more patient-centered outcomes while simultaneously improving system performance through reductions in avoidable hospital and emergency utilization. New AI-enabled predictive models create an opportunity for health systems to proactively identify high-need patients and deploy scalable digital solutions that reliably deliver the right care, to the right patient, at the right time, and in the right place.
