Babylon Announces Integrated Digital-First Chronic Condition Programs Offering Personalized Evidence-Based Care Within Single Interface

Babylon, a digital healthcare company announced today personalised, digital-first programs for high-risk members living with chronic conditions. The programs are offered as part of Babylon’s already existing integrated primary care model that collects and analyses in real time to monitor patients with multiple chronic conditions.  The program covers conditions such as: diabetes, hypertension, low back pain, prenatal care, anxiety, and depression. 

Babylon’s newest technology will allow for earlier intervention from care teams through more accurate patient monitoring. Personalised programs will include scheduling of regular patient touch points, and ensuring patients have accurate information on their condition, with care goals adjusted as needed.
