Biofourmis, a global leader in technology-enabled care delivery, and Lee Health, a not-for-profit health system in Southwest Florida, are set to launch the Hospital at Home model and expand their remote patient monitoring (RPM) program. Lee Health, known for its leadership in RPM, has achieved a 50% reduction in 30-day readmissions and operates one of the largest RPM programs in the U.S., with an average daily census of over 700 patients.
Under this partnership, Biofourmis will support Lee Health’s entire RPM service line while introducing a first-of-its-kind Hospital at Home program for Southwest Florida. This model provides hospital-level care at home, bringing necessary resources and medical teams to patients, leading to better patient satisfaction and clinical outcomes.
The Biofourmis platform enables continuous monitoring with features such as an integrated emergency response system, arrhythmia and fall detection, and continuous vital sign tracking, all accessible to the nursing team for 24/7 oversight. Jennifer Higgins, Chief Nursing Officer at Lee Health, emphasized that the Hospital at Home model enhances patient care by allowing patients to heal in their homes with professional support.
Biofourmis will also integrate with Lee Health’s Epic Electronic Health Record (EHR) system to ensure seamless care coordination and access to critical data for both acute and post-acute management. Additionally, FDA-cleared predictive analytics will help detect patient deterioration early, reducing alarm fatigue and improving nursing efficiency.
The program will also manage simultaneous health services, ensuring coordinated and efficient care.
