Drawing upon insights gleaned from the extensive COMPASS study spanning five years in North Carolina, StrokeCP adopts a data-centric approach to deliver effective interventions and tools for post-stroke care management.
Tailored Care Plans: Leveraging patient-specific information such as clinical background, demographics, medications, and social determinants of health, StrokeCP crafts personalised care plans supplemented with bespoke resources.
Remote Monitoring and Guidance: StrokeCP facilitates seamless communication between patients and caregivers, enabling remote monitoring and coaching to ensure continuous support.
Seamless EHR Integration: Through seamless integration with electronic health records, StrokeCP streamlines data capture and utilisation for enhanced efficiency.
Emphasis on Social Determinants of Health: StrokeCP addresses critical social factors influencing recovery, aligning with the burgeoning emphasis on social determinants of health (SDOH) in healthcare. In light of CMS's mandate to screen for five key SDOH risk factors, especially prevalent in underserved communities, StrokeCP's approach is particularly timely and impactful.
