Cigna to Cover Cleerly’s AI Heart Plaque Analysis

Cleerly, a leader in AI-based cardiovascular imaging, announced that Cigna will begin covering its Cleerly LABS Advanced Plaque Analysis on October 1, 2025. This move makes Cigna the latest major insurer to recognize the clinical and economic value of Artificial Intelligence Enabled CT Based Quantitative Coronary Tomography (AI-QCT) and Coronary Plaque Analysis (AI-CPA) technology in cardiovascular care.


With over 16 million covered lives, Cigna joins UnitedHealthcare and EviCore in offering access to Cleerly’s advanced imaging technology. Together with Medicare’s 2024 coverage decision and the introduction of a new CPT I code in January 2026, these approvals extend access to AI-QCT and AI-CPA technology to more than 61 million Americans.


Starting October 1, 2025, all Cigna health plans will cover AI-QCT/AI-CPA imaging for patients who meet specific clinical criteria: those with acute or stable chest pain but no known coronary artery disease (CAD) who are eligible for CCTA; patients whose CCTA results classify them as intermediate risk, or CAD-RADS 1, 2, or 3; and patients whose cardiac evaluation is negative or inconclusive for acute coronary syndrome (ACS).
