The platform, known as Journeys Metabolic HI-TRUST, is EHR-interoperable and streamlines metabolic care for population health initiatives by automating provider workflows and engaging targeted patient populations.
The core clinical component of the platform is a 4-month, physician-developed digital program designed to facilitate long-term behaviour modification and prevent metabolic diseases, including type 2 diabetes, NAFLD, metabolic syndrome, and obesity. This program leverages AI-driven behavioural profiling to offer a personalised nutritional experience. So far, 3,000 patients have benefited from the program, leading to significant improvements, including an average 8% body weight loss, 17% reduction in A1c levels, and 54% resolution of NAFLD, with a focus on enhancing HEDIS scores.
