Digital health technology company, Lark Health is collaborating with U.S-based Smart Meter to improve care and management of patients with type 2 diabetes across heath systems. The collaboration will see Lark Health’s diabetes management program, Diabetes Care combined with Smart Meter’s proprietary cellular-enabled iGlucose, glucose metre.
Lark’s diabetes care program provides patients with real-time benefits, narrowing the care gap for those with diabetes. The artificial intelligence (AI) care programme offers 24-hour unlimited text messaging care coaching. Additionally, it tracks medical appointments, A1c tests, kidney disease screenings, eye exams, flu shots, blood pressure tests, and cholesterol tests, as well as keeping track of patients medication adherence. Lark’s platform monitors glucose, activity, mood, nutrition, and sleep data, highlighting health trends and activity over time.
Smart Meter is a supplier of remote patient monitoring solutions. The partnership with Lark Health aims to enhance the ability of health systems to manage patients with Type 2 diabetes.
Smart Meter’s iGlucose glucose metre is designed to help patients quickly and easily test their blood sugar levels, automatically notifying the patient’s healthcare providers of any glucose readings. The patient's glucose data is transmitted from Smart Meter to Lark Health and to healthcare providers using iGlucose.
