Ochsner Digital Medicine, a leader in remote patient management, has teamed up with Humana Healthy Horizons, Louisiana's Medicaid plan, to provide digital health programs for members dealing with hypertension and type 2 diabetes. Starting from February 1, 2024, eligible Humana Healthy Horizons members can enrol in these remote programs, leveraging Ochsner Digital Medicine's expertise in utilising remote patient monitoring technology and personalised care plans.
The initiative aims to empower patients by allowing them to track vitals, medications, and progress remotely, receive personalised care plans tailored to their needs, ensure optimal medication adherence through clinician collaboration, and receive guidance for lifestyle modifications. Ochsner Digital Medicine's unique care team approach focuses on managing chronic conditions rather than just monitoring them, contributing to improved patient outcomes.
The partnership addresses the challenges of chronic disease management through digital solutions, emphasising the importance of remote patient management in effectively handling conditions like hypertension and type 2 diabetes.
