Saint Luke’s launches Digital AFib care program with Story Health

Saint Luke’s Mid America Heart Institute, in collaboration with Story Health, has launched a comprehensive program aimed at supporting patients with or at risk of developing Atrial Fibrillation (AFib). This new initiative is designed to provide continuous patient engagement and support between clinic visits, addressing various aspects of AFib care, including symptom management, accelerated treatment, stroke prevention, and ongoing patient support.


Despite being the most prevalent form of cardiac arrhythmia, AFib often goes underdiagnosed and undertreated, significantly raising the risk of stroke, particularly in older patients. The Story Health AFib program integrates seamlessly with clinical teams and employs a virtual, asynchronous care model to tackle these issues. It supports rapid treatment optimization, collects and monitors health data, and offers active support from specialized health coaches who guide patients and help reduce clinical workload.


Story Health’s AFib program represents an extension of its efforts to transform specialty care delivery, complementing its existing cardiology programs that have already benefited numerous healthcare organizations and tens of thousands of patients nationwide. Following its initial implementation at Saint Luke’s, the program will gradually roll out to other health systems. Dr. Sanjaya Gupta of Saint Luke’s emphasized the program’s potential to extend the reach of clinicians and elevate the overall level of patient care.
