With over 1.3 million adults in Ohio living with CKD, only 150,000 are aware of their condition, leading to delayed diagnosis and treatment. The collaboration aims to leverage the resources and reach of both organisations to increase awareness and early intervention, offering value-based kidney care across various lines of business, including Medicare Advantage, individual, and commercial group health plans.
The strategic partnership is poised to impact more than 10,000 Ohioans living with kidney disease. Strive Health, already serving over 100,000 patients nationwide, brings its proven value-based care model, resulting in a 42% reduction in hospitalisations and a 94% satisfaction rate among patients. Strive's approach combines technology-enabled interventions with seamless integration with local providers, and the partnership will extend these benefits to eligible Medical Mutual members. The initiative includes coordinated care, proactive management, and improved patient experience through dedicated care teams, educational resources, and support services.
