Teladoc Health Launches Person-Centered Virtual Care Platform for Connected Care

Teladoc Health has introduced Teladoc One, a new virtual care model designed to provide more personalized, coordinated healthcare by focusing on the individual rather than treating conditions in isolation. The platform brings together primary care, chronic disease management, mental health and acute care within a single connected experience, aiming to address the longstanding fragmentation that often forces patients to navigate multiple providers and digital health services on their own. The company says the new model is built to adapt continuously to each person's evolving healthcare needs while improving clinical outcomes and reducing overall costs.


At the center of Teladoc One is the company's Pulse intelligence engine, which combines clinical history with information from medical records, pharmacy data, claims, connected devices, eligibility records and patient engagement data. These insights help multidisciplinary care teams identify health risks earlier, coordinate interventions and recommend the most appropriate care pathway. Rather than simply referring patients elsewhere when additional care is needed, Teladoc Health's teams are designed to coordinate treatment across virtual services and local healthcare providers, ensuring continuity throughout the patient's care journey.


The platform supports patients through a combination of licensed clinicians, mental health professionals, registered dietitians, certified health coaches and dedicated care guides who help coordinate appointments, monitor progress and connect patients with specialists when necessary. An always-on AI assistant complements these care teams by providing reminders, gathering relevant health information, assisting with scheduling and maintaining engagement between clinician interactions. According to the company, this integrated approach reduces the burden on patients while helping clinicians deliver more timely and personalized care.


Teladoc Health also plans to link its compensation directly to performance under the new model by placing all of its program fees at risk, tying payment to clinical outcomes and total cost-of-care improvements across covered populations. Initially, Teladoc One will focus on patients with cardiometabolic conditions before expanding to additional populations over time. The company expects to begin rolling out the platform with select customers in September 2026, followed by broader availability in January 2027, as part of its strategy to deliver more connected, outcome-based virtual healthcare.
