Rimidi, a clinical management platform, has announced that TrueCare, a nonprofit community health centre, has successfully implemented the Rimidi platform to improve hypertension control through remote blood pressure monitoring.
With the support of a Health Resources and Service Administration (HRSA) Hypertension grant, TrueCare integrated Rimidi's platform into their operations. The platform enables patient-generated blood pressure data to be seamlessly incorporated into the clinical workflow through configurable decision support cards and alerts. TrueCare selected Rimidi because it integrates with cellular-enabled blood pressure cuffs, eliminating the need for Wi-Fi, smartphones, or apps. This is crucial for TrueCare, as the majority of their patient population falls below the federal poverty level.
The implementation involved designing a Remote Patient Monitoring (RPM) program workflow, configuring customised alert thresholds and automated text messaging, and providing clinical decision support to identify non-compliance with readings. TrueCare refers patients to the RPM program if they have uncontrolled blood pressure, and health coaches guide patients on using the cellular-enabled blood pressure device and educate them about medication, diet, and exercise.
The program started in January 2023 and has shown significant progress, with 70 enrolled patients and an average of 10 new patients added each month. Patients who graduate from the program have achieved controlled blood pressure, and adjustments to medications are made if the goal is not met.
TrueCare has observed high compliance with an average of 30 monthly readings per patient. The program has also helped identify White Coat Hypertension, where patients have high blood pressure in the clinic but not at home. TrueCare plans to expand the program, especially with the opening of an Older Adult Health Care location, and integrate Rimidi with their Epic OCHIN EHR to provide a comprehensive view of patient data.
