Uptiv Health and Lyn Health have joined forces to introduce Uptiv360, an integrated solution merging infusion therapy with chronic care management. This partnership leverages Uptiv Health's tech-enabled infusion platform and Lyn Health's focus on providing whole-person care to poly-chronic patients. Through an app-based onboarding process, patients can seamlessly connect with a care team, undergo evaluation during infusion visits, and access Lyn Health's virtual chronic care management, including behavioural health services, within 48 hours. The collaboration aims to address the comprehensive healthcare needs of chronic patients, emphasising the intersection of mental health and chronic diseases.
Uptiv Health, launched in August with $7.5 million in funding, has been actively enhancing patient engagement. In December, the company introduced a patient-engagement application enabling communication with clinical teams through chat and video sessions, managing appointments, digital check-ins, and accessing medical history. Uptiv's app also facilitates billing and payment services, care planning, coordination, and community and caregiver services. The partnership with Lyn Health not only expands Uptiv's offerings but also introduces a groundbreaking approach to screening behavioural health issues for infusion patients and connecting them with professional therapists within a 48-hour timeframe.
