Walgreens partners with value-based care platform Pearl Health

Walgreens is teaming up with Pearl Health, a company that provides a technology platform for primary care practices involved in value-based care arrangements. This collaboration aims to expand value-based care options through primary care providers. 

Pearl Health's platform assists clinicians in identifying patients who may require extra support, like those with chronic conditions or recent hospital discharges, using data visualisation and recommendations. The tools simplify patient information and monitor metrics such as readmission rates and wellness visit attendance.

In this partnership, Walgreens will offer services like prescription fulfilment, immunisations, medication adherence, diagnostic testing, and care-gap closure. They will also assist patients transitioning from the hospital to their homes. 

The goal is to improve healthcare services, particularly in underserved communities, by focusing on proactive and coordinated care. Both companies are excited about the potential benefits for Medicare patients and beyond.
