From insight to impact: Why analytics alone won’t fix value-based care

Author:

Value-based care doesn’t have a data problem. It has an action problem.

Many health care organizations over the last decade have invested heavily in analytics platforms, predictive models and reporting tools designed to support and improve value-based care performance. Despite the unprecedented access to data, there is still a fundamental challenge: turning insights into meaningful action. Data has no value until it is activated.

Organizations that have made the greatest progress toward value-based care don’t necessarily have the most data. What sets them apart is that they have learned how to make their data work for them by embedding intelligence directly into clinical workflows.

Analytics remain essential, but analytics alone is no longer enough.

Data abundance paradox

Most health care organizations have access to an incredible amount of data, but too much information arriving through too many disconnected channels can be challenging.

Population health, healthcare economics, actuarial, quality, finance and care delivery teams often work toward the same goals while relying on different data sets, insights and workflows. Each team may have valuable insights, but without a shared view and coordinated way to act, care opportunities may be missed.

Shifting the focus from analytics to action

The next generation of analytics is less about reporting and more about enabling impactful action.

Sifting through volumes of data to find and interpret actionable insights takes a lot time and resources. At Optum, the focus is on AI-enabled prioritization, interoperability and workflow integration. This approach is designed to help care teams identify high-priority actions and act on them quickly.  The goal is simple: the right action for the right patient at the right time.

Build a clearer understanding of population needs

Effective population health management begins with knowing where to focus.

A broad view of a population can help organizations recognize patterns, assess areas of opportunity and better understand the factors influencing health outcomes. These factors may include utilization history, comorbidities, care gaps and social needs.

AI-supported insights can help teams evaluate this information more efficiently and surface the factors that may require closer attention. The goal is not to replace clinical expertise or human judgment. Human oversight remains essential.  Instead, AI insights are designed to help teams move beyond “analysis paralysis” and shine a light on care opportunities where action may make a meaningful difference.

This clearer view can also give teams across the organization a shared foundation for setting priorities and aligning resources.

Prioritize the people who may benefit most

A population may include thousands of people who are potentially at risk, but some individuals face more immediate or serious risk to their health than others. Without a structured way to prioritize engagement, organizations risk spreading resources too broadly or responding too late.

Identifying a care opportunity is only the beginning. Organizations must also translate population-level insights into focused interventions that support better health.

Rather than applying the same approach across an entire population, care teams can use available data and insights to identify which patients are most likely to benefit from earlier or more intensive support. These groups can then be organized into targeted cohorts and connected with the appropriate care programs or resources.

This approach helps health care organizations focus on delivering the right care to the right patient. By creating a stronger connection between population health strategy and the individual care that follows, teams can act earlier for the patients who need it most and prevent health events before they happen. The goal is simple: better care for each patient's individual needs.

Reduce friction between insight and engagement

Insights only create value when teams can act on them.

Navigating disconnected systems, searching for relevant information and coordinating across multiple teams can add friction to already complex care delivery processes. When population insights, care opportunities and workflows are connected, teams can move more seamlessly from identifying a need to taking an action.

Optum Value Connect helps bring prioritized opportunities into care management workflows so teams can begin engagement with richer context. Care managers can use that information to guide conversations, develop personalized care plans and coordinate support based on a patient’s needs. When additional needs are identified, other members of the care team, such as pharmacists, physicians and care coordinators, can become part of the process. This creates a more connected approach to care, where outreach is informed by broader population insights but tailored to the individual patient.

Keep teams aligned around the patient's needs

Coordinated care depends on more than identifying and engaging the right people. It also requires visibility across the broader care team.

When care plans, interventions and next steps are shared across connected workflows, team members gain a clearer understanding of each patient’s needs and the support already provided. This shared view can help reduce duplicate outreach, improve coordination and create a more consistent experience across the care journey.

Most importantly, it creates a feedback loop between action and insight. Information captured through care delivery can help organizations assess the impact of their programs, refine their approach and continue working to improve how they prioritize opportunities.

Create an operating model for action

Better data is only part of the answer. Organizations also need an operating model that helps teams use information consistently and coordinate the actions that follow.

Connecting population health insights with prioritization and care management workflows can help teams:

  • Understand the needs and opportunities across a population

  • Focus resources on the patients who may benefit most

  • Coordinate engagement across the appropriate care teams

  • Capture information that can inform future decisions

Bringing these capabilities together can help population health, quality, finance and care delivery teams work from shared insights while contributing their individual expertise.

The objective is not simply better analytics. It is a more connected approach.

The future belongs to actionable intelligence

As value-based care continues to evolve, AI and technology will play a larger role in identifying risk, surfacing care opportunities and helping teams prioritize action. But technology alone won’t determine success.

Leaders in the next era of value-based care will be those who build operating models where intelligence is embedded directly into workflows, enabling clinicians, providers and operational teams to make meaningful action while outcomes can still be influenced.

The next era will not be defined by who has the most data, or even the most sophisticated platform. It will be defined by who can transform insight into coordinated action consistently.

Intelligence creates potential, and action creates value.
