Basalt Health Raises $20M to Scale AI-Powered Post-Acute Care Platform

Basalt Health has raised $20 million in Series A financing, bringing its total capital raised to $24.5 million. The company will use the funding to scale its AI-driven platform for automating patient transitions and admissions into post-acute care facilities.

Founded in 2024 by CEO Ben Hackett, a former Accolade executive, Basalt automates the processing of referrals between acute and post-acute care providers. The platform ingests referrals from sources including fax, secure web portals and direct EHR interfaces, then evaluates patient information against clinical admission criteria, staffing capabilities, bed availability and payer requirements.

Basalt is addressing a process that remains heavily dependent on manual document review. Approximately 14 million patients enter post-acute care annually, while hospital case managers typically send clinical dossiers averaging 70 pages to multiple facilities when seeking an appropriate placement.

At Lifepoint Health, Basalt reported that its platform reduced median chart processing time by 86%, from 8.5 minutes to 1.2 minutes per referral.

The new capital will support expansion across 111 enterprise markets by the end of 2026. Basalt plans to extend its deployment into 49 additional Lifepoint Health markets and scale across 62 ScionHealth hospitals nationwide. At ScionHealth, the technology will manage incoming post-acute referrals across a network of more than 6,000 practitioners.

Beyond admissions intake, Basalt plans to extend its platform into additional workflows, including acute discharge planning, payer prior authorization and benefits coordination.

The company is positioning its technology as an automation layer between acute and post-acute providers, aiming to reduce the manual work associated with reviewing lengthy and unstructured patient records. By accelerating referral assessment and placement decisions, the platform is intended to support patient transitions while helping hospitals and post-acute facilities manage capacity and admissions workflows more efficiently.
