Longevity Health and Innovaccer Expand Partnership to Launch Enterprise AI Center of Excellence

Innovaccer and Longevity Health have announced the expansion of their partnership to establish an enterprise-wide AI Center of Excellence, building on measurable financial and operational outcomes achieved through prior collaboration. The initiative will be powered by Innovaccer’s Gravity™ platform and is intended to scale artificial intelligence capabilities across clinical, operational, and financial workflows.

The expansion follows a year of implementation during which Longevity Health deployed Innovaccer’s cloud-based platform within its Azure environment to unify disparate data sources. The system consolidated clinical, claims, pharmacy, ADT, and MDS data across more than 200 facilities, creating a centralized data infrastructure to support value-based care delivery.

Using this unified data layer, Longevity Health reported a reduction in inpatient costs of nearly 20%, generating approximately $1 million in savings over 12 months. The organization also improved utilization by nearly 10%, particularly across chronic care management programs targeting high-acuity conditions such as diabetes, end-stage renal disease (ESRD), and Alzheimer’s disease.

Operational improvements were supported by real-time dashboards and embedded point-of-care intelligence tools, which enabled care teams to standardize workflows, improve coordination, and intervene proactively in patient care. These capabilities were particularly relevant for Longevity Health’s focus on elderly populations with complex clinical needs.

The newly announced AI Center of Excellence will extend these capabilities by introducing enterprise-scale AI tools and applications, including:

  • Development of AI-powered chatbots and internal assistants

  • Creation of retrieval-augmented generation (RAG) agents

  • Deployment of custom business intelligence and risk models

  • Natural language interfaces for data access

  • Enhanced productivity and financial performance initiatives
